Helping people live well at home

Support at Home is about enabling people to continue living well in their own home, with the right support around them.

At LINK, we focus on supporting people to remain safe, happy and healthy at home – while staying connected to their community, services and the things that matter most to hem.

Our role is to deliver the day-to-day services that help people stay engaged, maintain routine and feel confident living independently.

How LINK supports your clients

LINK provides a range of services that work together to support independence and connection in everyday life.

Our social support programs create regular opportunities for people to stay engaged with others and their community. Through group activities, outings and individual support, we help people maintain routine, build confidence and remain connected.

Transport plays an important role in making this possible. We provide reliable and consistent transport that enables people to attend appointments, access services and take part in activities – supporting participation beyond the home.

We also deliver domestic assistance and home maintenance to help people maintain a safe, comfortable and functional home environment, supporting their ability to live independently.

A more tailored approach to social support

LINK’s service model is strengthened by a qualified social work capability, supporting a more considered and individualised approach.

Our social workers contribute to how services are designed and delivered, particularly where clients may require additional support to engage or where needs are more complex.

This includes supporting:

  • Clients experiencing social isolation or reduced confidence
  • More tailored approaches to engagement and participation
  • Alignment between services and individual goals
  • A consistent, person-centred approach across service delivery

We align with care plans and individual goals, delivering services in a way that is consistent, reliable and responsive to change. Communication is clear and practical, supporting continuity of care and a positive client experience.

You continue to lead care planning, coordination and oversight. LINK supports the services that help bring those plans to life.

Supporting care managers

We understand the practical demands of care management and the importance of services that work in real life.

Services need to be easy to organise, reliable in delivery and meaningful for the person receiving them. They need to be used, valued and able to adapt as needs change.

LINK is set up to support this – providing services that engage clients, support independence and deliver consistent outcomes over time.

Our social work capability provides additional support where engagement, wellbeing or complexity requires a more tailored approach.

LINK’s helpers are trained to look out for you. They are encouraged to notice things around the house, or to look out for a changes in your own wellbeing. We might be the first people to notice something going wrong. Where we can, we will take steps to make sure you are alright and getting the support you need from the whole community of care – engaging with your family, carers and favourite health practitioners.

When to refer to LINK

LINK is well-suited when clients would benefit from increased connection, participation and support to remain independent at home.

You may consider referring to LINK when a client:

  • Is becoming socially isolated or less engaged in their usual activities
  • Would benefit from regular social support or structured group programs
  • Needs support to rebuild confidence after a change in health or circumstances
  • Is not currently engaging with existing services
  • Requires reliable transport to attend appointments, activities or community programs
  • Would benefit form support to re-establish routine and community connection
  • Needs assistances to maintain a safe and manageable home environment

How to refer to LINK

Referring to LINK is straightforward.

You can refer a client by contacting our team directly to discuss their needs, goals and preferred supports. We will work with you to confirm the most suitable services and ensure alignment with the client’s care plan.

Once a referral is received, LINK will confirm service requirements, arrange commencement in line with client preferences, and maintain communication with you regarding service delivery and any changes.

We aim to make the process simple, responsive and easy to manage.
